Sustained Hope, LLC provides specialized home care services designed to help people challenged with daily-living activities due to age-related illness, neurotrauma and other disabling conditions.

We offer SPRINT, an innovative patient transition and navigation program, which helps improve post-stroke patient survivorship and quality of life.  Our SPRINT-enabled neurotrauma centers and Smart Rehabilitation Homes would help achieve improvement in patient outcomes while reducing care costs.

POST-DISCHARGE PATIENT CARE
IS FRAGMENTED

Follow-up communication, monitoring, and care delivery for patients recently discharged from the hospital are inadequate at best. 

• Post-discarge care centers are fragmented silos;
• Discharged patients are often left to recover and improve without any monitoring or assistance; many patients receive no follow-up calls;
• Care providers lack age-in-place technologies & products.

As the industry shifts towards preventative care, post-discharge patient care must take a proactive approach to effective care delivery instead of the reactive quick-fix that is currently employed.

Sustained Hope provides private, technology-assisted care services to seniors needing help with daily-living because of memory challenges due to neurodegenerative diseases and those who need post-discharge care after a medical trauma. 

We help patients transition from the hospital to their residence (through our innovative SPRINT program), and match them with the caregiver that’s right for them.  Sustained Hope’s mission is to use SPRINT to reduce readmissions and improve safety and quality of life, while reducing care cost.

We’ve received positive feedback from prospects, existing clients, caregivers and assisted living facilities. SPRINT has traction at two reputable hospital systems.

Partnerships
Navigation Tracker™ is the base architecture of SPRINT. We’ve also partnered with telehealth technology providers.

Noteworthy
We have a Provisional License from the GA-Department of Community Health-Healthcare Facility Regulations Division (Feb 2017). 

GA-DCH called us to potentially send services to help monitor, treat, rehabilitate, and provide caregivers to Puerto Rico hurricane victims 
(Sept 2017).

WHAT'S NEXT

Business Development
With the technology developed and ready to be deployed, Sustained Hope will continue pushing forward with new business development.

Customer Acquisition
Benchmarking surveys have identified the internet—including websites and social media—as the best marketing tool. Therefore, Sustained Hope plans to acquire referrals through online media. Leads are currently generated through internet lead generation services. The lead-to-client conversion rate is currently ~5%. Customers will also be cultivated through referrals from physicians, hospital systems, assisted living facilities, caregivers and through word of mouth.

Smart Rehabilitation Home Development
Concurrently, Sustained Hope will continue to secure strategic partnerships with hospitals and technology partners to create and launch the best-in-class smart rehabilitation homes.

Pilot Study Launch
Most importantly - Sustained Hope will proceed with launching the pilot study to further validate the potential of SPRINT/Navigation Tracker™.
